Colonoscopy cuts risk of colorectal cancer death in half, study shows ...

Doctors have proved for the first time that removing polyps during a screening colonoscopy not only prevents colorectal cancer from developing, but also prevents deaths from the disease, according to an analysis to be published Thursday.

By studying the fate of 2,602 high-risk patients in a 23-year time period, a team of medical investigators led by researchers at Memorial Sloan-Kettering Cancer Center in Manhattan concluded colonoscopy cuts the risk of colorectal cancer death in half.

?The evidence demonstrates a striking reduction in death,? said Dr. Sidney J. Winawer, lead investigator of the National Polyp Study and a Sloan-Kettering gastroenterologist.

?By finding and removing polyps, there was a 53 percent reduction in the death rate,? Winawer said, adding that colorectal cancer is the only malignancy that can be prevented through a screening technique.

For years, doctors knew colonoscopy helped reduce the risk of colorectal cancer when precancerous growths _ polyps _ were removed.

The problem: No one had ever quantified by how much colonoscopy reduced the risk of death.

Knowing that figure, doctors said, would help bolster another observation: Cancer statistics were revealing a noticeable decline in colorectal cancer incidence over the last two decades. Doctors believed _ but were uncertain _ that the decrease could be attributed to colonoscopies.

Dr. Ann Zauber, senior author of the study, which is reported in Thursday?s New England Journal of Medicine, said she and her team proved colonoscopy cut the death rate in half by comparing colorectal cancer deaths of study participants to those of people in the general population.

In the group of 2,602 patients, 25.4 deaths from the disease would have been expected. Instead, there were only 12.

Despite advances in screening, colorectal cancer remains a potent cause of cancer mortality in the United States, and only 30 percent of people 50 and older undergo the test, according to data from the National Cancer Institute.

The institute also estimates more than 140,000 new cases will be diagnosed this year and that more than 50,000 people will die of the disease.

Yet, even in the face of colonoscopy?s benefits, it has come under fire, Winawer said.

?The effectiveness of colonoscopy has been challenged recently, along with mammography, the PSA and CT scanning for lung cancer,? Winawere said. ?So, yes, these questions have been raised, but you can?t ignore the major benefits from this form of screening.?

Two years ago, Drs. Alfred Neugut and Benjamin Lebwohl of Columbia University argued colonoscopy was no more effective than sigmoidoscopy, another screening technique.

Last year, relying on New York State Health Department data, Dr. David Bernstein, chief of gastroenterology at North Shore University Hospital in Manhasset, reported on a colonoscopy risk factor: possible colon perforations during the procedure.

Bernstein said while perforations are rare, colonoscopy is best performed by skilled specialists.

Zauber, meanwhile, noted the new results should inspire renewed confidence in the procedure. ?Our findings provide strong reassurance that there is a long-term benefit to removing these polyps and support continued recommendations of screening colonoscopy in people over age 50.?

Using the Joint Tactical Radio System (JTRS) Handheld, Manpack, Small Form Fit (HMS) two-channel networking radio (AN/PRC-155), General Dynamics C4 Systems recently completed the first demonstration of secure voice and data communications via the Mobile User Objective System (MUOS) satellite-communications waveform.

The demonstration used an AN/PRC-155 manpack radio - running the MUOS waveform software - to transmit encrypted voice through a MUOS-satellite simulator to the MUOS ground station equipment that will soon be deployed in Sicily.

MUOS is a military satellite communications system that will enable secure, mobile networked communications worldwide, in even the most-austere environments. Development of the MUOS waveform remains on track for completion in the third quarter of 2012. By year-end, the MUOS capability will be available on the AN/PRC-155 manpack radio, the first MUOS terminal that will be available to soldiers.

"As one of the demonstration participants, I was pleased to talk over the MUOS network using the PRC-155 radio as it will be equipped for soldiers.

"When fully deployed, the MUOS system will greatly increase much-needed tactical satellite capacity, in terms of both communications bandwidth and the numbers of users," said Brigadier General Michael Williamson, Joint Program Executive Officer, Joint Tactical Radio System.

"This demonstration proved that the JTRS HMS PRC-155 two-channel radio is ready to provide the portable link necessary to put the power of MUOS into warfighters' hands," said Chris Brady, vice president of Assured Communications for General Dynamics C4 Systems.

"General Dynamics developed the MUOS waveform using the PRC-155 manpack radio and it will be the first MUOS communications terminal used by soldiers. With two channels in one radio, a soldier can use one channel for line-of-sight SINCGARS and SRW waveforms, and bridge to the second channel using the MUOS satellite system for unprecedented, dedicated global communications reach."

General Dynamics C4 Systems is the prime contractor of the JTRS HMS radio program, which includes the two-channel PRC-155 networking radio, the PRC-154 Rifleman Radio and several other small form fit networking radios.

The JTRS HMS program stipulates that two qualified manufacturers compete for full-rate production contracts, enabling greater affordability for the government. General Dynamics and Thales Communications, Inc. will manufacture the PRC-154 Rifleman Radio.

The months-long battle in Congress over extending a payroll tax break ?certainly caused damage? to Republicans because it ?muddled? the differences between the parties over the issue, says House Budget Committee chairman Paul Ryan (R) of Wisconsin.

Early Thursday morning, House and Senate leaders agreed on a bipartisan deal that will extend, through the end of the year, a 2 percentage point reduction in 160 million workers? Social Security taxes. The rate will stay at 4.2 percent of earnings, rather than revert to its usual 6.2 percent. Republicans in December had balked at a long-term extension of the tax break, but after intense criticism they agreed to back a short-term extension through February.

House Speaker John Boehner announced earlier this week that the House GOP leadership would no longer insist that the $100 billion cost of the tax break be paid for with spending cuts, because the Senate was unwilling to go along with the cuts. [Instead, the lost revenue for Social Security will be offset by money from the general fund, effectively adding to the federal deficit.?

?The payroll tax deal, from a political perspective, certainly caused damage because it muddled the differences [between the parties]. It got us down into a skirmish where the differences got muddled, which is what I think the president loves,? Representative Ryan said at a Monitor-hosted breakfast for reporters on Thursday.?

The influential House Republican directed his fire at the Democratic-controlled Senate. ?It is a tough issue because they had to compromise,? Ryan said. ?It is very frustrating that they, meaning the Senate, weren?t willing to cut $100 billion [from government spending] over 10 years.?

The House Budget Committee chairman added: ?The idea of running against a dysfunctional, do-nothing Congress is ... part of [President Obama's] campaign strategy." Ryan charged: "In order for that to happen, the Senate has to be your enabler to make sure Congress does nothing.?

Pentagon details downsizing of US forces in Europe

The number of troops stationed in Europe will be reduced by more than 11,000 by 2017, defense officials said on Thursday.

Families affected by the moves were notified on Thursday, Stars & Stripes?reported on its website.

About 80,000 U.S service members are currently deployed throughout Europe.

Two infantry brigades, the 170^th Infantry Brigade and the 172^nd Separate Infantry Brigade, each?with 3,850 soldiers, will be moved off the European continent in the next two years, defense officials said in a news release.

The 170^th, which is?redeploying to Baumholder, Germany, from Afghanistan, will begin moving with troops' families this summer. The 172^nd, now in Afghanistan, first will redeploy this summer to Germany, then moving with troops' families in the summer of 2013.

The Pentagon also announced the closure of Army garrisons in Schweinfurt, Bamberg and Heidelberg by fiscal year 2015.

Reductions in U.S. forces in Europe were announced earlier this month as part of Defense Secretary?Leon Panetta's plan to slash $487 billion from the Pentagon budget.?Which?units would move,?however,?had not been announced.

In a news conference?Thursday with German Defense Minister Thomas de Maiziere, Panetta said a sizable force will remain in Europe.

?Despite these changes,? Panetta said, according to Stars & Stripes, ?over 40,000 U.S. troops will still be based in Germany, training at state-of-the-art facilities.?

Maiziere said?the move goes?from??quantity to quality.?

?The withdrawal of U.S. troops from Germany is no complaint to me because a lot of troops will remain in Germany,? Maiziere said.

China leader-in-waiting Xi woos and warns U.S. (Reuters)

WASHINGTON (Reuters) ? China's leader-in-waiting Xi Jinping on Wednesday offered deeper cooperation with the United States on trade and security, citing Iran and North Korea, but called on Washington to heed Beijing's demands on contentious "core interests" such as Tibet.

Xi, who is almost sure to become China's next president in just over a year, laid out his views on ties with the United States in the keynote speech of his visit to Washington.

His message was dominated by reassuring vows of more balanced economic ties and more international cooperation. But he also stressed Beijing's impatience with U.S. policies on Taiwan and Tibet -- issues where many Chinese citizens expect their leaders to show they will stand up to foreign pressure.

"The world is currently undergoing profound changes, and China and the United States face shared challenges and shoulder shared responsibilities in international affairs," Xi told a ballroom crowded with U.S. business executives, academics and policy-makers involved with China.

"We should further use bilateral and multilateral mechanisms to enhance coordination between China and the United States on hotspots, including developments on the Korean peninsula and the Iran nuclear issue," said Xi.

Xi's visit to the United States this week has given him a chance to boost his international standing before his likely promotion to the head of China's ruling Communist Party later this year and president in early 2013.

Although his speech revealed no new policies, his offer to work with Washington on dealing with Iran and North Korea could ease U.S. worries that Xi (pronounced like "shee") could take foreign policy in a more hawkish direction.

Despite Xi's comments, both sides continued to prod each other on some of the issues that have fanned friction.

President Barack Obama took aim at China's trade policies, saying he will not stand by when American's competitors "don't play by the rules." [ID:nL2E8DFEUZ] And members of Congress pressed Xi on China's detentions of human rights activists.

The Chinese vice president offered his own warnings about Tibet and Taiwan, two territories where Beijing fears that its claims could be undermined by Western pressure.

"History demonstrates that whenever each side handles relatively well the issues bearing on the other side's core and major interests, then Sino-U.S. relations are quite smooth and stable. But when it is the contrary, there are incessant troubles," he said.

Washington should "abide by the one-China policy and take concrete actions to oppose Taiwanese independence," he said.

"We also hope that the United States will truly implement its recognition that Tibet is part of China and its vow to oppose Tibetan independence, acting prudently in issues concerning Tibet," he added.

Tensions over Chinese control of Tibet have flared in past months when a succession of protests and self-immolations have exposed volatile discontent. Chinese officials have blamed those tensions on separatists or supporters of the Dalai Lama, the exiled Buddhist leader of the region.

In early 2010, the Obama administration's decision to move forward with proposed arms sales to Taiwan triggered vehement criticism from Beijing, including warnings of sanctions against U.S. companies involved in the sales.

Those warnings petered out, but Xi made clear that Taiwan remains an acute concern for Beijing.

Xi acknowledged the Obama administration's recent strategic "pivot" towards Asia, which will see a more mobile U.S. military presence, but warned Washington not to push too far.

"China welcomes the United States playing a constructive role in promoting the peace, stability and prosperity of the Asia-Pacific region, and at the same time we hope the U.S. side will truly respect the interests and concerns of countries in the region, including China," said Xi.

Mounting maritime tensions in the Asia-Pacific region is likely to be one of the main geopolitical stress points in the coming decade.

"SPEAKING FRANKLY"

Xi, 58, is poised to become China's next leader after a decade in which it has grown to become the world's second largest economy while the United States has fought two wars and endured the deepest recession since the Great Depression.

Xi met with House of Representatives Speaker John Boehner and Senate Majority Harry Reid on Wednesday morning and after his speech headed to Iowa for the next leg of his trip, which finishes later this week in Los Angeles.

Boehner's office said his staff delivered a letter on the case of Gao Zhisheng, a dissident and human rights lawyer imprisoned in China. The House Speaker also expressed disappointment at China' veto of a U.N. Security Council resolution on Syria.

Many U.S. lawmakers complain that China's yuan currency is significantly undervalued, giving Chinese companies an unfair price advantage that helped lift the U.S. trade deficit with China to a record $295.5 billion in 2011.

U.S. Treasury Secretary Geithner said on Wednesday that Beijing is letting its currency rise, but not fast enough.

"We think they have some ways to go, we would like them to move more quickly," he told a congressional panel.

Xi said currency reforms already taken by Beijing helped boost U.S. exports to China to more than $100 billion in 2011 and has significantly reduced China's overall trade surplus.

"China has become the United States' fastest growing export market," Xi said. "The trade surplus as a proportion of GDP has been falling from over seven percent to two percent, at a level internationally recognized as reasonable."

In a bid to ease trade gap concerns, Chinese commercial delegations have been travelling across America signing orders while Xi visits. In Iowa, a delegation signed deals to buy 8.62 million tones of soybeans from the United States.

Xi repeated Beijing's call for the United States to eliminate restrictions of exports of high-technology civilian goods, which he said would help bring trade into balance.

Beijing has given the Obama administration a framework for promoting two-way trade and investment, he said.

"Speaking frankly, an important aspect of addressing the imbalance in Chinese-U.S. trade is the United States' own economic policies and structural adjustment," he said.

Turkish premier discharged after second surgery (AP)

ANKARA, Turkey ? Turkey's state-run news agency says Prime Minister Recep Tayyip Erdogan has been discharged from hospital a day after undergoing what officials said was a "second and final" intestinal surgery.

The Anadolu Agency said Erdogan left hospital late on Sunday and was expected to rest at his home in Istanbul.

The 57-year-old Erdogan vanished from public life for nearly a month after an initial surgery on Nov. 26 for non-cancerous intestinal polyps.

His office said he had a second, successful half-hour procedure on Saturday but gave no other details.

It said Erdogan was well and was expected to resume work within the next week.

Analysis: Obama's course correction shifts dynamic (AP)

WASHINGTON ? President Barack Obama's opposition is now the divided one.

For three weeks of heated rhetoric, Republicans cast the president's new rule that religious schools and hospitals must provide insurance for free birth control to their employees as an attack on individual liberty. The contentious issue united recently fractured Republicans, Catholic bishops and religious groups while badly splitting Democrats who feared an election-year fallout.

Obama's leading GOP rivals ? Mitt Romney, Rick Santorum and Newt Gingrich ? had sensed a political opening and were relentless in criticizing the president.

Obama caved to the pressure on Friday, announcing a compromise that allows employees at religious-affiliated institutions to get free contraception directly from health insurance companies rather than employers who say it would violate their long-held convictions.

Almost immediately, Democrats who had disagreed with the White House backed the revised policy. So did Sister Carol Keehan, president and chief executive officer of the Catholic Health Association of the United States and a crucial player in both this debate and the fierce fight over Obama's health care overhaul law. The head of the nation's bishops called it a good first step but reserved final judgment.

The once formidable coalition against the president had splintered. Factions that had stood with the GOP cautiously backed Obama's midcourse correction. It was a necessary policy change that reversed the political dynamic.

"After the many genuine concerns that have been raised over the last few weeks, as well as, frankly, the more cynical desire on the part of some to make this into a political football, it became clear that spending months hammering out a solution was not going to be an option, that we needed to move this faster," Obama said in announcing his retreat and compromise.

The comment was a clear acknowledgment that his administration needed to move away quickly from an all-consuming battle that pitted Obama against the Catholic Church, hardly the fight a president wants to pick when he's seeking another term.

"At the end of the day, Church one, White House zero," said Sara Taylor Fagen, a Republican strategist and White House political adviser in George W. Bush's administration.

The policy and the fury underscored the difficulty for the administration in implementing elements of Obama's sweeping health care law, which remains highly divisive nearly two years after it became law and within months of the Supreme Court rendering its judgment sometime in late spring. It reflected the nervousness among congressional Democrats and candidates who want to avoid alienating working-class voters and suburban women critical to their fate this November.

The initial policy had drawn opposition from Democrats such as Sens. Joe Manchin of West Virginia, Bob Casey of Pennsylvania and Rep. John Larson of Connecticut, all Catholic and all facing re-election this year. Challenging the administration was Tim Kaine, former chairman of the Democratic National Committee, the Senate candidate in Virginia and a Catholic who worked with Jesuit missionaries in Honduras.

There was palpable relief among several with the president's announcement.

"I am pleased that the White House has taken further steps to ensure that all women have access to affordable contraception and to ensure that religious organizations will not be asked to violate their beliefs in the process," Kaine said in a statement.

Larson praised the president for finding a "path forward to provide coverage to everyone while addressing the conscience concerns of religiously affiliated organizations."

Manchin and Casey held off on a final assessment, saying they were looking at the details.

Before announcing the decision, Obama called Keehan, Planned Parenthood President Cecile Richards and Cardinal-designate Timothy Dolan, head of the nation's Roman Catholic bishops. Keehan was key in a fight that turned out to be health care redux. Two years ago, her support for the health care law in the face of the bishops' opposition helped sway several conservative and moderate anti-abortion Democrats to back the legislation, votes that lifted the bill into law.

On Friday, shortly after Obama's White House appearance, Keehan issued a statement: "The Catholic Health Association is very pleased with the White House announcement that a resolution has been reached that protects the religious liberty and conscience rights of Catholic institutions. The framework developed has responded to the issues we identified that needed to be fixed."

Mo Elleithee, who worked on Hillary Rodham Clinton's presidential campaign, said the administration can argue that in a dysfunctional, highly partisan Washington, the president found a way to compromise.

"I think what he did today was one of those great examples of good policy and good politics," Elleithee said. "A lot of people wanted to turn this into a war between women's rights and religious liberties. As long as that narrative existed, it wasn't good for the White House. The solution allows him to show commitment to contraceptives for women and sensitivity to religious liberty and maybe most importantly compromise while finding a win-win solution."

Republicans vowed to continue to fight, looking to push legislation in the House and possibly the Senate on religious liberty. Even if the measures stand little chance, it would force Democrats to cast a vote in an election year. They also signaled that they will argue the issue on economic terms, contending that free birth control coverage will mean increased health care premiums for many Americans.

"The administration promised during debate on the health care law that constitutional rights would not be infringed and that costs would go down," said Senate Minority Leader Mitch McConnell, R-Ky. "We now know that's not true."

Obama already knows his opposition in that battle.
